Domiciliary Home Care
Rise and Shine Care treat all service users as individuals and show the dignity and respect that we would expect ourselves.
An initial assessment is conducted before any domiciliary home care is provided. From this an Individual Care Plan is prepared. This enables Rise and Shine Care to develop an all round comprehensive view of the individuals physical, spiritual and emotional needs. The initial assessment also ensures the correct and most appropriate level of care for each individual.
q The Individual Care Plan is carried out with the service user and / or a member of their family.
q The Individual Care Plan is a comprehensive assessment of all aspects of their health, personal and social care needs and considers preferences and choices.
q The Individual Care Plan sets out any problem that the individual has and the proposed action to deal with it.
q A key-worker will be designated to each individual. As a team Rise and Shine Care will be directly responsible for your individual, personal and social care needs as identified within the Individual Care Plan.
q The Individual Care Plan will be regularly reviewed.
q Regular risk assessments on self-caring skills and autonomy will ensure maintenance of independence as far as is reasonably practicable.
q The resulting Individual Care Plan will be made available where required.
As a result Rise and Shine Care offers a standard of domiciliary home care that is recognised and appreciated by both service users and their families.
